Cleveland Home Health Agency’s nurses provide professional, compassionate care in the privacy of home. Working with the patient’s physician, an individualized plan of care is developed, based on the patient’s specific needs. Services must be medically reasonable and necessary for the patient’s diagnoses and for treatment of the illness or injury.
A registered nurse is always available 24 hours a day,
7 days a week.
Skilled nursing services include:
- Wound Care
- Infusion (excluding blood / blood products)
- Injections
- Pain Management
- Assessment and education regarding disease processes
- Assessment and education regarding home safety
- Medication education
- Ostomy care
- Foley catheter care / changes / irrigation
Pediatric nursing services include :
- Assessment and education regarding growth and development
- Assessment of chronic condition
- Weight monitoring
- Phototherapy treatment of newborn jaundice
- IV or IM antibiotics
- Apnea Monitors
- Diabetic teaching
- Ostomy, Catheter, Gastrostomy care / education
- Routine ongoing labwork
- Parental Education regarding care of high risk infants of children
